What are Blink Forms?
Blink Forms are digital forms that can be created and managed directly within the Blink platform using the Form Builder in the admin portal. Admins can create custom forms through a drag-and-drop interface to address variety of use cases, such as incident reports, change of details, expense submissions, and more. Form submissions are stored and accessible within Blink for admins, with the option to export results.
Blink supports embedding and sharing external forms such as those created using Microsoft Forms, Google Forms, Typeform, and other providers—but Blink Forms are native and their results are directly stored and reportable in Blink, which isn’t the case for those other tools.
How to create a Blink Form
Please note that this feature will need to be enabled by our support team prior to use and is only accessible to Admins. If you are an Admin and do not see the Forms option in the Admin portal, please contact our Support Team or your Customer Success Manager. All admins have access to forms that they've been added as a collaborator of and have access to create new forms but only Organization Admins have access to all forms.
New Form
Open Blink on the web or desktop app and select the Admin portal.
In the Content Studio section to the left, select the Forms option
Click Create Form and select "from blank" in the dropdown
Add the desired title and description for your form as a whole at the top. You'll also have the option to add a form logo in the circled section below.
For best results, we recommend 460px wide by 70px high
Add the page title and description if desired in the box shown
Add your desired questions (see available options here)
To add another page, click the Add Page button to the left
To change the form settings, click the Form Settings option and select the options desired in the menu to the right to fully customize your users' experience
Once you're happy with your form, click the Preview option at the bottom to see what your users will see.
After publishing your form, we also recommend submitting the form as a test to make sure the form submission process flows as desired and to test your recipient inbox rules.
To finalize all changes, click Save then navigate back to the main Forms page by clicking "Forms" to the left.

Template
Open Blink on the web or desktop app and select the Admin portal.
In the Content Studio section to the left, select the Forms option
Click Create Form and select "from template" in the dropdown
This will open a view where you can access two different template libraries. The "Templates" section will show any templates that you or other admins have created for your organization. The Template Gallery section will show a number of already-created form structures that our team has made to help you get started. Select the desired template from the available options.
Edit and update the template as desired by adding/removing/changing questions, restructuring the form, and making any changes that may be necessary to make the existing template work best for your organization.
Once you're happy with your form, click the Preview option at the bottom to see what your users will see and test out any settings you may have added.
After publishing your form, we also recommend submitting the form as a test to make sure the form submission process flows as desired and to test your recipient inbox rules.

Form structures and question options
Form structures are customizable to meet your organization's needs. You can create forms with as many or as few questions as you'd like in a variety of formats. Admins can also add a form logo, page headers and descriptions, and more.
At present, we support all of the following question types:

This can use labels/text, stars, or smileys
Checkboxes
You can set minimum and maximum responses for these via the question settings to the right.

You can set minimum and maximum responses for these via the question settings to the right.

You can allow multi-file uploads in the question settings to the right of this.

Single-Line Input
Can be formatted to be text, date, time, date and time, number, phone number, etc.

Profile Field
This auto-populates with data from the user's Blink profile. Please note that this field will not work on forms that you've set to be submitted anonymously. Can auto populate the First Name, Last Name, Email, Mobile Phone, Work Phone, Employee ID, Department, Company, Location, or Job Title. At present, this feature does not support auto-filling custom profile fields.
There are also some question options that are will be coming in the future. If you're interested in using these or others, please reach out to our support team or your customer success manager and we'll be happy to assist with creating this question type in your form.
Profile Field Options
Auto-populated dropdown with existing field options based on your user profiles in Blink. This will show users options for Department, Location, Company, Job Title, or Team

Recipient options
Once you've created the form, you can customize your email recipients to ensure that the desired people receive these form submissions.
Go to the Forms section of admin
Select the three dots to the right and choose the "Email Notifications" option
On this screen, you can customize your recipients
Admins can customize this to where submissions from users who meet certain criteria will be sent to a specified recipient. This can be set using profile details on the user's account on Blink or submissions that match certain values as well:
You can set as many recipients as desired and add separate rules for each recipient email is desired. Additionally, admins can opt to have a copy of the submission sent to the submitter's email and/or the submitter's manager
Sharing forms with your organization
Blink forms can be shared to the Feed, in the Hub, via Journeys, or with a shareable link directly from the form builder itself.
Share to the Feed
There are a few different ways to share Blink forms to the Feed:
In the admin portal, click the three dots to the right of the form you'd like to share and select the "share to Feed option".
While adding the form to the Hub, you can select to share it to the Feed when it is published on the Hub.
You can use the "copy link" option shown below and share that link in the body of a Feed post.
Share in the Hub
In the Admin portal, navigate to the "Hub" section.
Click "Add content" and select "Form" from the dropdown
Set the form provider to "Blink" and select your desired form from the dropdown below.
Select the groups to share this content with and what section to add it to on the Hub.
Optionally, you can also share this form to the Feed from this view, set the form as a quick link, and/or make the form "protected content"
When you're happy with your selections, click the "Share" button to publish your form to the Hub.
Share via Journeys
Blink Forms can also be added as steps in the Journeys you create for your organization. For more information about the Journeys feature, please check out this article.
To add a form to a Journey, copy the link from the form itself or you can copy the Hub share link if you've already shared this item to the Hub.
Share via link
From the Form builder screen in the Admin portal, click on the three dots to the right of the form you'd like to share, then select "copy link". This will provide you with a shareable link that you can send to users in your organization to access the form.
Reviewing Submissions
In addition to the PDF files that will be sent to the designated recipients, your admins are also able to see previous submissions directly in Blink. Admins can also export all the submissions received to have all of the form data in one single spreadsheet. Submissions will remain accessible in the Admin portal indefinitely.
Go to the Forms Section of Admin
Click on the desired form
This will open the Form Responses view with all recent submissions
From there, your admin can select the Export option for a single submission or click the "Export All" option to pull a report of all submissions for this form.
FAQs
How many recipients can I add to receive a copy of a form submission? | There is currently no limit to the number of recipients. |
Can forms be submitted anonymously? | Yes, you can set your form to only be submitted anonymously by changing the "Form Privacy" setting in the form settings to Anonymous. It currently defaults to include user details. Please note that any profile field data that you've set to auto-fill (first name, last name, etc.) will be left blank if the form is set to be anonymous. |
Can you limit form responses? | You can set the form to limit all users to only one response by selecting the corresponding option in the Form Settings in the Form Builder view. |
Why don't I see the Forms option in Admin? | This is a feature our team has to enable for organization upon request. If you don't presently see the option, please reach out to our support team and we'll be happy to assist. |
Who can create a form on Blink? | Group Admins, Content Moderators, and Organization Admins can all create forms. Organization Admins can see all forms while other admins will only be able to see forms they've created or have been added as a collaborator on. |
How do I edit a form? | Admins can edit forms by clicking the three dots to the right of the form in the Forms view on the admin portal and selecting "edit form". From there, you can make the desired changes just as you would when creating a form. Once you save the changes, they will be live and visible for users submitting the form. |
How do I add a collaborator? | You'll need to be an Owner or Editor of the form to do this. In the Forms section of the Admin Portal, click the three dots to the right of the desired form, select "Manage collaborators" and enter the desired admins in the text box. |
How can I export responses? | You can export responses to a form by choosing the "export all" option on the Form Submissions page in admin for the desired form. The "Export All" option will provide a CSV file with all submissions or, if you only need to export one response, click the "export" option on the desired response and you'll have the option to export that file as a CSV, PDF, or JSON file. |
Why can't I view the attachments in the submission? | This is usually an access issue. Make sure first that you are signed into Blink in the same browser that the attachment is being opened in. You'll also need to make sure you're a collaborator and/or listed as a form recipient for the desired form. If you're still getting an error or experiencing any issues, please contact our support team. |
How long are submissions visible in the Admin portal? | These submissions remain accessible indefinitely and do not expire. |
How can I create my own template? | Click on the "create form" dropdown and select "manage templates". Click the "create template" button and build your form as you would when creating a new form. |
What templates are available? | We have a variety of templates accessible in the "Template Gallery" section of the form builder with more being added. At present, these templates include a Uniform Order Form, Cleaner Form, 7/15/03/90 Day Check-in, Near Miss Report, Holiday Request Form, Pre-boarding Form, and more. |
Who can be added as a collaborator on forms? | Any user can be added as a collaborator with Editor or View Only access as desired. |
What's the difference between the "Templates" and "Template Gallery" options? | "Templates" are just for your organization: those are templates your admins have created in Blink. "Template Gallery" shows templates that our Blink Team has created to provide a starting point for some of the most common form use cases. |
